In what marks the first chapter of a thrilling trilogy, the year is 2037. A volcanic eruption on the fictional island of Equaland results in severe environmental pollution and skyrocketing rates of organ failures amongst the island’s citizens. Lin Zhekang (Zhang Zetong), a devoted son scraping by on the edges of society, resorts to fighting illegal boxing matches to fund a lung transplant for his ailing mother Lin Aiqin (Jesseca Liu), who has been diagnosed with silicosis. Along the way, he crosses path with Ren Xin (Cheryl Chou), a doctor whose icy exterior masks a fiercely compassionate heart.
As the crisis deepens, the shortage of organs leads to an increase in kidnappings and illegal organ trade on the black market. Among the syndicates profiting from the chaos is organ trafficking ring “Straits Hall”. One day, Lin Zhekang helps Zhang San (Jeremy Chan), a driver for the syndicate, defend himself from an attempted robbery – only to witness Ren Xin’s younger sister Li Yina get kidnapped. He is attacked when he tries to save her.
Fearing her son will be dragged deeper into the underworld, Lin Aiqin plans to flee the island with him – but is abducted by Straits Hall during the escape. Ren Xin turns to her friend, police officer Gao Shang (James Seah), and together they use Zhang San as a lead to locate the whereabouts of the missing abductees.
Meanwhile, disgraced doctor Xiao Wenze (Li Nanxing) is determined to advance his research on artificial human wombs and lab-grown organs. As he seeks to recover lost research, Wenze entrusts Guo Longjing (Desmond Tan), a member of the Straits Hall, to locate his former research assistant, who turns out to be Lin Aiqin. While reluctant, the latter agrees to hand over the sole surviving research sample, Zeta, on one condition: Wenze must keep Zhekang safe. With Zeta in his possession, Wenze prepares to rebuild his career. 
As the crisis unfolds, Zhekang, Ren Xin, Zhang San and Gao Shang join forces to save the abductees. But just as the pieces begin to fall into place, Equaland’s supervolcano erupts once more, plunging the island into a state of emergency…
